- Copy each service the case plan names, the person it names the service for, and the page. Add a service only when the plan names it. Write no sentence of the application.
- What the lawyer must review
- You write the application. Section 675(1)(B) includes a plan for assuring that the child receives safe and proper care and that services are provided to the parents, the child and the foster parents in order to improve the conditions in the parents' home, facilitate return of the child to his own safe home or the permanent placement of the child, and address the child's needs while in foster care, including a discussion of the appropriateness of the services that have been provided under the plan. Section 671(a)(15)(A) makes the child's health and safety the paramount concern in determining reasonable efforts, and subparagraph (B) requires reasonable efforts to preserve and reunify families except as subparagraph (D) provides. The list does not decide that efforts are excused.
