## Key facts

- Kind: Form. A form to complete, or a guide to completing an official one.
- Practice area: [Personal Injury & Compensation](https://ailawframework.com/precedents/personal-injury-and-compensation.md)
- Jurisdictions: New South Wales
- AI level: Red. You write the text. The tool writes nothing into the document. Where permitted, it may check, clarify or collect data, and anything it checks or collects is verified.
- Law stated as at: 2026-10-23
- Status: Maintained. Checked on 2026-10-23 against the official form at ucprforms.nsw.gov.au and the 2 authorities it cites, for New South Wales.

## When to use it

Use when particulars of a personal injury claim are prepared for filing in a New South Wales court on UCPR Form 14. Compensation to relatives proceedings use Form 15.

## Used for these tasks

- [Draft pleadings or points of claim from the medical chronology](https://ailawframework.com/practice/personal-injury-and-compensation/draft-pleadings-or-points-of-claim-from-the-medical-chronology.md)
- [Prepare a damages or statutory-entitlements schedule](https://ailawframework.com/practice/personal-injury-and-compensation/prepare-a-damages-or-statutory-entitlements-schedule.md)

## How AI may be used

The tool writes nothing in the particulars that are filed, and it may only extract a working list of injuries, expenses and wage figures already on the file.

**AI may:**

- Extract a working list of injuries, treatment dates and out-of-pocket amounts from records already on the file
- List wage records by employer and period for the lawyer who writes the economic particulars
- List documents that the form says are served with the particulars

**AI must not:**

- Must not write the filed particulars, including the injuries, disabilities, care claim or economic loss
- Must not sign the form or file it
- Must not describe the plaintiff's pain or capacity in words that are not already in a report the lawyer has accepted

**The lawyer checks:**

- The form is Form 14 version 2 from the current UCPR list. It cites UCPR 15.12
- Headings on the form are injuries, continuing disabilities, out-of-pocket expenses, domestic assistance or attendant care, loss of income, loss of earning capacity and future economic loss, and any other amounts
- The list of documents served includes hospital, medical and expert reports, accounts, workers compensation accounts and employment records. Give reasons where a document cannot be served with the statement
- The legal representative or the party signs
- This guide is red because the particulars are filed. The form has no affidavit and no statutory declaration
- Health, wage and other personal information stays in a firm-controlled tool. Check confidentiality under ASCR r 9 and APP 11, and whether any breach is notifiable under the Privacy Act 1988 (Cth).
