Signed authority for one hospital to release the emergency, inpatient, outpatient and imaging records of named attendances, plus a letter asking the hospital which access path and fee it requires. It is not a subpoena and it is not a statutory declaration.

## Key facts

- Kind: Form. A form to complete, or a guide to completing an official one.
- Practice area: [Personal Injury & Compensation](https://ailawframework.com/precedents/personal-injury-and-compensation.md)
- Jurisdictions: Victoria, Queensland, New South Wales
- AI level: Yellow. You and the tool draft together, with you directing. You check every statement you rely on.
- Law stated as at: 2026-10-23
- Status: Maintained. Checked on 2026-10-23 against the 5 authorities it cites, for Victoria, Queensland and New South Wales.

## When to use it

Use for each hospital the client attended for the injury or for an earlier condition the claim needs. If the hospital will release records only on its own form, or only under a public-sector information request, use that path and keep this authority as the client's instructions.

## How AI may be used

The tool may fill the hospital and the attendance dates from the intake, and the lawyer checks the consent and the access path before anything is sent.

**AI may:**

- Fill one authority for each hospital named in the intake
- List the attendance dates the client gave, and leave unknown dates blank
- Ask the hospital, in the covering letter, which request path and fee it requires

**AI must not:**

- Must not call this authority a subpoena or a freedom-of-information application
- Must not include a mental-health admission unless the client has ticked that box
- Must not send records to an unapproved tool or an ordinary personal email

**The lawyer checks:**

- The client has identified the hospital and the attendances, including whether a mental-health admission is included
- The hospital's own form or a public-sector access application is used if the hospital requires it
- Health information is stored under APP 11 and ASCR r 9. A loss likely to cause serious harm is assessed under the Notifiable Data Breaches scheme
