## Checking plan

**Verify against sources**

Include only documents the lawyer has selected.

- What the AI does: In the firm-controlled tool, list the title, date and source of each supporting document the lawyer has chosen for the notice of claim. Do not write the notice.
- What the lawyer does: The lawyer writes any verification in the notice. The tool lists the supporting documents only.
- Who reviews: Lawyer competent in this practice area
- Basis: Checked against the linked sources on 23 October 2026. Confirm the current instrument before reliance.

## Checks the lawyer makes

1. Include only documents the lawyer has selected.
2. Match tax returns and group certificates to the three years immediately before the injury, as section 275(8) states.
3. Include the notice of assessment unless the claimant is a worker with a terminal condition or a dependant.
4. The lawyer writes the statutory declaration required by section 275(5). Nothing is generated into it.

## Notes

- Section 275(8) requires the notice to be accompanied by the documents supporting the claim, including hospital and medical reports other than reports obtained by or for the insurer, income tax returns, group certificates and other documents for the three years immediately before the injury, documents for out-of-pocket expenses, and, other than for a worker with a terminal condition or a dependant, the notice of assessment. Section 275(5) requires any statement in the claimant's personal knowledge to be verified by statutory declaration. Section 275(6) requires a genuine offer or the reasons an offer cannot yet be made. The declaration and the offer are written by the lawyer. This list is of the documents only.
- If this work is filed, use the Queensland Courts generative AI page. That page names Supreme Court Practice Direction 5 of 2025 and District Court Practice Direction 12 of 2025, both on the accuracy of references in submissions, and it also names Planning and Environment Court Practice Direction 7 of 2025 and Land Court Practice Direction 4 of 2025. Supreme Court Practice Direction 14 of 2024, subparagraph 16(l), is about expert evidence in criminal proceedings. The direction PDFs were not opened, so their operative text is not set out here.
- Keep health and wage records in a firm-controlled tool. ASCR r 9 and APP 11 apply. A breach may be notifiable under the Privacy Act 1988 (Cth).

## Practical guide

Jurisdiction: Queensland: the documents that accompany a notice of claim under section 275

**Deliverable**: A list of titles and dates for the documents accompanying the Queensland notice of claim.

**Inputs**

- Documents the lawyer has selected
- Notice of assessment
- Three years of earnings documents

**Example**: Illustrative example: The lawyer has selected the notice of assessment and two years of tax returns. The tool lists those documents and flags the third year as missing. It does not write the declaration.

**Stop and escalate when**

- The lawyer has not selected the documents.
- The tool has written a sentence of the declaration or the offer.
