## Checking plan

**Device adverse event**

Show whether the event sits on the sponsor path or the facility path, and which minimum fields are missing.

- What the AI does: From the incident note, list the device, the date of use, the facility, whether a sponsor has already reported, and which of the minimum fields are missing. Do not lodge a report.
- What the lawyer does: The lawyer decides whether the facility or the sponsor must report, and lodges any report.
- Who reviews: Responsible lawyer with competence in the matter
- Basis: Practical workflow informed by the linked primary sources; the traffic light and working checks are editorial guidance, not a court classification.
- Why: The level reflects what the work is, the consequence of an error, and how much of the output the lawyer must independently re-derive.

## Checks the lawyer makes

1. From 21 March 2026 the page says facility reporting is mandatory. Confirm the page still says that before relying on the date.
2. Do not report twice. A report already made through the incident scheme is enough on this page.
3. Do not restate the regulations. The compilation was not opened.
4. The lawyer lodges the report. The index does not.
5. This is not the clinical incident-reporting map and it is not a sentinel-event review.

## Notes

- These are suggested working checks. Confirm the required checking with the responsible lawyer before work starts; the checking approach does not establish permission to use AI.
