| How AI is used | From the enrollment and the immunization record, list the student's grade, each required vaccine the file shows, the date of the first day of school, and whether a medical exemption is on the file. Do not decide that the student may attend, and do not write an exclusion notice. |
| What the lawyer must review | You decide whether the record meets the requirement of the student's State, and you write any exclusion notice. New York's school-vaccine page says children in day care and pre-kindergarten through grade 12, in public, private, and religious schools, must receive all required doses on the recommended schedule to attend or remain in school, unless they have a valid medical exemption. A medical exemption is allowed when a child has a medical condition that prevents the child from receiving a vaccine. There is no nonmedical exemption. Within 14 days of the first day of school or day care, a parent must show proof of up-to-date vaccinations or provide a valid medical exemption. A child who is unvaccinated or overdue must receive at least the first dose of each required vaccine within the first 14 days, and subsequent doses within a 14-day period of when they are due. The page lists diphtheria and tetanus toxoid-containing vaccine and pertussis vaccine, hepatitis B, measles mumps and rubella, polio, and varicella for day care, pre-kindergarten, and school; Tdap for grades 6 to 12 and meningococcal conjugate vaccine for grades 7 to 12, with an additional MenACWY dose for grade 12 on or after the 16th birthday; and Haemophilus influenzae type b and pneumococcal conjugate vaccine for day care and pre-kindergarten. The Department of Health's schools immunization page says Public Health Law section 2164 requires the annual school immunization survey, including for pre-kindergarten programs housed in a school. Another State's list, grace period, and exemption rule can differ. Use that State's health department page before applying this list outside New York. |

## Checking plan

- What the AI does: From the enrollment and the immunization record, list the student's grade, each required vaccine the file shows, the date of the first day of school, and whether a medical exemption is on the file. Do not decide that the student may attend, and do not write an exclusion notice.
- Basis: Checked against the linked sources on 22 October 2026. Confirm the current instrument before reliance.

## Notes

- The 14-day periods and the vaccine list on this page are New York's. Another State uses its own statute.
- The page allows a medical exemption and no nonmedical exemption. Do not add a nonmedical exemption from another jurisdiction.
- Dose counts for each vaccine, beyond the grade rules stated on the page, were not opened.
