## Checking plan

**Source-by-source verification**

Immediate telephone notice, and the scene is left undisturbed except for the steps the page lists.

- What the AI does: From the incident notes, list whether the file describes a death, a serious injury or illness, or a dangerous incident, the time the business became aware, and what has been done with the scene. Do not make the telephone report.
- What the lawyer does: The lawyer decides that the incident is notifiable, that the telephone report is made immediately, and what may be done at the scene.
- Who reviews: Responsible lawyer with competence in the matter
- Basis: Practical workflow informed by the linked primary sources; the traffic light and working checks are editorial guidance, not a court classification.
- Why: The level reflects what the work is, the consequence of an error, and how much of the output the lawyer must independently re-derive.

## Checks the lawyer makes

1. A death, a serious injury or illness, or a dangerous incident is reported immediately on 13 10 50. Reports can be made 24 hours a day. The page says some dangerous incidents must be notified even if no one is injured. It does not list those types, so the lawyer uses the legislation the page links.
2. The scene is not disturbed until an inspector arrives or gives a direction. The page allows action to assist an injured person, remove a deceased person, make the site safe, assist with a police investigation if directed, or comply with an inspector request.
3. First aid is provided. The incident is recorded in the register of injuries. The workers compensation insurer is notified within 48 hours of becoming aware of a workplace injury.
4. The page states a maximum penalty for failing to notify of $50,000 for a body corporate and $10,000 for an individual. The lawyer makes the report. The note does not.

## Notes

- These are suggested working checks. Confirm the required checking with the responsible lawyer before work starts; the checking approach does not establish permission to use AI.
