Paragraph 2 says the note commences on 3 February 2025. Open the District Court PDF. It does not add a separate formula. **What is the effective date?** Paragraph 2 says the note commences on 3 February 2025. The face of the note is dated 18 December 2024. **What should the disclosure say?** Follow the adopted SC Gen 23 paragraph for the document you are filing. Paragraph 13 is the affidavit disclosure.
